You may request a replacement ballot by emailing your request to elections@ufcw3000.org with the following information:

• Legal first and last name

• Work location

• Home mailing address

• Last four digits of your Social Security number or date of birth, for verification purposes

If your eligibility is verified, a replacement voter packet will be mailed to the address provided in your email.

If the Election Committee determines that you are not eligible, the committee will respond in writing with its decision.
